Getting Your Library Card

We are excited that you are interested in applying for a library card with the Siloam Springs Public Library!

You need to be 18 years or older to apply for a library card. Children over the age of five are allowed to have their own library as long as a parent or a legal guardian has their own library card. Please also note that the owner of the library card has the sole information about the materials they check out. A waiver must be filled out and signed by the cardholder if they want information released to someone else.

You will need to show 2 forms of ID when your application is processed at the library:
Current Driver’s License or another state or federal issued Photo ID card
Proof of current physical address (checkbook, utility bill, receipt, etc.)
[P.O. Box addresses must be verified with proof of current physical address]

Adult Library Card Application (18 and over)

Minor Library Card Application (Ages 5-17)

You can complete your form online with e-signatures or fill out the form at the library. If you fill out the form online and submit it, bring two forms of ID listed above to complete the process of your application.

Card Costs

Inside city limits or paying personal residence City utilities:
              Free    (bring City bill for verification)
Outside city limits (not on City utilities):
             Adults: $10.00 (Payable by cash, check, credit card, or debit card)
             Child: $5.00 (Payable by cash, check, credit card, or debit card)

Card Renewals

Cards are renewed every year, not replaced. Please notify staff of any changes that are made to your account.
There is a $3.00 fee for replacement cards.

Monday & Thursday 9:00 a.m. - 7:00 p.m.
Tuesday, Wednesday, & Friday 9:00 a.m. - 5:00 p.m.
Saturday 10:00 AM - 2:00 PM
