Getting Your Library Card
We are excited that you are interested in applying for a library card with the Siloam Springs Public Library!
You need to be 18 years or older to apply for a library card. Children over the age of five are allowed to have their own library as long as a parent or a legal guardian has their own library card. Please also note that the owner of the library card has the sole information about the materials they check out. A waiver must be filled out and signed by the cardholder if they want information released to someone else. (Parents can request their children sign a waiver giving them permission to view materials).
You will need to show 2 forms of ID when your application is processed at the library:
Current Driver’s License or another state or federal issued Photo ID card
Proof of current physical address (checkbook, utility bill, receipt, etc.)
[P.O. Box addresses must be verified with proof of current physical address]
You can complete your form online with e-signatures, download the PDF and bring the completed form into the library, or fill out the form at the library. If you fill out the form online and submit it via e-mail, bring two forms of ID listed above to complete the process of your application.
